Back-to-School Forms
Welcome to the Back-to-School Forms Hub for the 2026–2027 academic year at YingHua International School!
In order to ensure a safe, smooth, and successful start to the school year, all families must complete the following forms no later than August 28, 2026. These forms help us keep your child safe, connected, and supported—both day to day and in emergency situations.
If your information changes during the year, please email communications@yhis.org.
Required Forms
Submit the phone numbers and email addresses to receive emergency alerts and school-wide notifications. Please note this is not necessary to complete if you registered last year. One online form per family.
Upload your child’s Universal Health Record (UHR) and Immunization Record, both completed and signed by a healthcare provider. You may also submit any applicable Allergy or Asthma Action Plans or other special medical forms. One online submission per student.
Submitting this form permits our nurse to assist your child when she/he isn't feeling well. Parents of children in kindergarten through Grade 5 should print and return one per student.
Opt-Out: Student publicity release form
YingHua International School may use photographs, audio/video recordings, or student work to promote school programs in print and digital formats.
No action is required if you grant permission.
Complete and return hard copy if you do not want your child’s name, image, or voice to appear in school publicity.
The New Jersey Nonpublic School Textbook Program provides free textbooks to eligible students attending private schools. Parents of students in Kindergarten through Grade 5 who are interested should complete and return a hard copy.


Emergency Contact & Authorized Pickup Form
Please provide key emergency contact information and list all adults authorized to pick up your child(ren) from school. Submit one online form per family, unless different contacts apply for individual children.
Submit your permission for YHIS to provide a Google Workspace for Education account to your child. Required for access to school-managed email, Google Docs, and other core tools. One online submission per student.
